POSITION TITLE: RN DIRECTOR OF QUALITY ASSURANCE

POSITION SUMMARY

The Director of Quality and Performance Improvement is responsible for overseeing and directing the home health and hospice quality assurance (Q/A), performance improvement (P/I), clinical education, pre-authorization, telehealth and document control activities of the agency

The Director of Quality and Performance Improvement is also responsible for developing, implementing and achieving annual goals and objectives; coordinating work within the department and other departments; ensuring compliance with operating policies and procedures and outside regulatory requirements and directly supervising assigned staff

POSITION RESPONSIBILITIES: 

  • Supports the Agency mission by providing health care and related services to those who are striving to achieve their highest possible level of independence, quality of life, health and comfort.
  • Develops/maintains education for Performance and Quality Improvement programs for home health and hospice.
  • Assures timely completion, accuracy and submission of the plans of care, CTI, OASIS and HIS.
  • Monitors and assures compliance with all deadlines for regulatory submissions
  • Assures pre-authorization is obtained in a timely fashion.
  • Assures data integrity and flow of data needed to complete documentation of client care and billing.
  • Develops and reports on Q/A and P/I program goals incorporating Lean concepts.
  • Pursues knowledge of Performance & Quality Improvement, legal, medical, licensing, certification.
  • Develops an annual Q/A program.
  • Monitors Performance & Quality Improvement activities and assures corrective actions.
  • Assures that regular record reviews are completed according to agency procedures.
  • Reviews documentation regarding software updates; plans for and assures completion of education and implementation of process changes.
  • Assures effective facilitation and appropriate activity of the Utilization Review, UR Sub and Audit Committees.
  • Assures development, coordination and presentation of training programs which reflect best practice protocols; addresses areas identified as needing improvement, changes in regulations or agency practices.
  • Monitors effectiveness of clinical education programming and initiates activities to maximize effectiveness.
  • Tracks data related to employee competence and use of standard best practice protocols and initiates education activities in cooperation with supervisory staff to optimize staff performance.
  • Assures adequate record-keeping for clinical education activities.
  • Acts as resource for clarification of documentation and reimbursement requirements, organizational procedures and QIO guidelines.
  • Completes all licensure applications and takes the lead in all licensure and Medicare survey visits.
  • Participates in Board of Directors and facilitates Advisory Group of Professional Staff.
  • Negotiates contracts and agreements.



POSITION TITLE: RN DIRECTOR OF QUALITY ASSURANCE

POSITION REQUIREMENTS:

  • Graduate of a school of nursing
  • Current license to practice as a professional nurse in the state of CT
  • Minimum of five years of experience in management and supervision in a in a homecare agency
  • Minimum of eight years of experience in nursing
  • training in the application of quality improvement initiatives.
  • Minimum of one year of focus on Performance & Quality Improvement
